We are seeking a dedicated Physician Assistant to join our established primary care practice in the Bronx, NY. This full-time position provides an exciting opportunity to offer comprehensive care to diverse patient population focusing on chronic care management, sick visits, medication management, gynecological exams, and tracking preventative care. In this role , you will be an integral part of the patient care team, ensuring that individuals receive high-quality, patient-centered services. You will work collaboratively with physicians and other healthcare professionals to manage ongoing treatment plans, monitor patient progress, and implement preventive care strategies to enhance long-term health outcome.

Key Responsibilities:
- Manage chronic conditions and provide follow-up care
- Conduct sick visits and create appropriate diagnosis and treatment plans
- Perform gynecological exams and promote preventive care
- Oversee medication management and monitor patient outcomes
- Maintain accurate and timely patient documentation
- Collaborate with the healthcare team to create comprehensive care plans
Qualifications:
- Active Physician Assistant License in New York
- Team player who takes initiative and thrives in a collaborative environment
- Ability to work autonomously while maintaining accountability
- Proficient in timely and accurate charting
