Job Description:
We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our team in Waco, TX. The ideal candidate will possess strong clinical skills in home health care and be committed to providing high-quality patient-centered care. As a Home Health RN, you will play a vital role in assessing patient needs, developing personalized care plans, and ensuring the delivery of healthcare services in the comfort of patients’ homes.

Key Responsibilities:
– Conduct comprehensive patient assessments to identify clinical needs and develop individualized care plans.
– Administer medications and treatments as prescribed by physicians.
– Monitor patient progress and adjust care plans as necessary.
– Educate patients and their families on disease management, treatment plans, and health maintenance.
– Collaborate with physicians, therapists, and other healthcare professionals to deliver coordinated care.
– Maintain accurate and timely documentation of patient care and progress in compliance with regulations and standards.
– Ensure patient safety and adhere to infection control protocols.
– Provide compassionate care and emotional support to patients and their families.

Qualifications:
– Valid Registered Nurse (RN) license in the state of Texas.
– Minimum of 1-2 years of experience in home health nursing.
– Strong clinical and critical thinking skills.
– Excellent communication and interpersonal skills.
– Ability to work independently and manage time effectively.
– Valid driver’s license and reliable transportation.
