Job Title: Home Health RN Case Manager
Location: Lewisville, TX
Employment Type: Permanent, Full-Time

Job Summary:
The Home Health RN Case Manager is responsible for overseeing and coordinating patient care in the home setting, ensuring quality and continuity of services. This role involves working closely with patients, families, physicians, and other healthcare professionals to develop and implement individualized care plans. The RN Case Manager will provide direct patient care and ensure compliance with all regulatory requirements and policies.

Key Responsibilities:
– Conduct comprehensive assessments of patients’ health status, including physical, psychosocial, and environmental factors.
– Develop, implement, and regularly update individualized care plans in collaboration with the patient, family, and healthcare team.
– Provide direct nursing care, including wound care, medication administration, and patient education, ensuring adherence to established protocols and standards.
– Coordinate and oversee care delivered by home health aides, therapists, and other healthcare professionals.
– Monitor and evaluate patient progress, adjusting care plans as necessary to achieve desired outcomes.
– Ensure compliance with all federal, state, and local regulations, as well as agency policies and procedures.
– Serve as a resource and advocate for patients and their families, providing support and guidance as needed.
– Educate patients and families about health conditions, medications, and self-care techniques to promote independence and optimal health.

Qualifications:
– Current and unrestricted RN license in the state of Texas.
– Minimum of 2 years of nursing experience, preferably in home health or case management.
– Bachelor’s Degree in Nursing (BSN) preferred.
– Current CPR certification.

Skills:
– Excellent clinical, assessment, and critical thinking skills.
– Strong organizational and time management abilities.
– Exceptional communication and interpersonal skills.
– Proficiency in electronic medical records and home health care software.
– Valid driver’s license and reliable transportation.
– Maintain accurate, timely, and detailed patient records and documentation.
