Schedule: Monday – Friday, 8:30am to 4:30pm (No weekends)

Job Overview:
We are seeking a compassionate and experienced Nurse Practitioner to join our family practice team at a mission-driven community health organization in Philadelphia, PA. This temporary-to-permanent role offers a stable career path in primary care, providing comprehensive healthcare services including evaluation, diagnosis, and management of acute and chronic conditions, as well as OB care. The position emphasizes collaborative care with nursing and administrative staff, accurate medical documentation, and a focus on preventive care and patient education to improve patient outcomes.

Key Responsibilities:
– Deliver high-quality primary care services in an outpatient family medicine setting, including OB services.
– Evaluate, diagnose, and manage both acute and chronic health conditions.
– Coordinate patient care effectively with the nursing and administrative staff.
– Maintain accurate and timely medical documentation using the EMR system.
– Promote preventive care and patient education to improve overall health outcomes.

Qualifications:
– Active Pennsylvania medical license required.
– Minimum of 3 years of experience in Family Practice, Urgent Care, or a related setting.
– Experience in providing OB services is preferred.
– Strong interpersonal and communication skills.
– Commitment to providing quality care and supporting community health initiatives.

Patient Load: Average of 10-15 patients per day with full support staff.
