Licensed Clinical Social Worker (LCSW)

Job Responsibilities:
– Provide clinical social work services, including psychosocial assessments, individualized treatment plans, and counseling to residents and their families
– Practice social work within long-term care (LTC) and skilled nursing facilities (SNFs), as well as select clinics
– Collaborate with interdisciplinary teams to ensure comprehensive care
– Advocate for residents’ needs and access to community resources
– Document and maintain accurate records in compliance with policies and regulations

Qualifications:
– Licensed Clinical Social Worker (LCSW) with an active Indiana license
– Experience in LTC, SNF, or clinical settings preferred
