Job Responsibilities:
– Perform Start of Care (SOC) and OASIS assessments
– Conduct recertification and resumption of care visits
– Provide wound care and supervisory visits
– Document visits in Homecare Homebase EMR
– Coordinate care with physicians and the care team
– Educate patients and caregivers on care plans
– Follow agency policies, state regulations, and infection control standards

Qualifications:
– Active RN license in Texas
– 1+ year of home health experience
– Experience with SOC and OASIS assessments
– Familiar with Homecare Homebase EMR preferred
– Valid driver’s license
