Job Responsibilities:
– Perform patient assessments, recertifications, and wound care
– Monitor patient progress and maintain accurate documentation
– Supervise and support LPNs and HHAs
– Collaborate with the care team on patient care plans
– Educate patients and families on health and treatment
– Ensure compliance with healthcare regulations and quality standards

Qualifications:
– Active Indiana RN license
– Experience with OASIS documentation required
– Experience with wound care, start-of-care assessments, and recertifications preferred
– Strong organizational and administrative skills
