We are hiring an experienced Registered Nurse to serve as a Home Health Case Manager for patients throughout the Garland area. Job Responsibilities:
– Complete OASIS assessments for admissions, recertifications, resumptions of care, and discharges
– Provide comprehensive case management and care coordination for assigned patients
– Conduct supervisory visits and ensure compliant, high-quality care delivery
– Coordinate care with physicians and interdisciplinary team members to ensure continuity of care
– Maintain accurate and timely documentation in the electronic health record (EHR) in accordance with regulatory standards
– Educate patients and caregivers on care plans and ongoing needs

Qualifications:
– Active Texas Registered Nurse (RN) license
– 1+year of home health experience (or hospice)
– Strong knowledge of OASIS standards
– Valid driver’s license with a reliable driving record
