Job Responsibilities:
– Complete patient intakes and assessments.
– Administer medications and treatments according to provider orders.
– Monitor and document changes in patient health and communicate concerns promptly.
– Support individualized care plans and promote patient well-being.
– Assist residents with daily living needs (bathing, dressing, feeding, mobility).
– Maintain accurate, thorough medical records and documentation.
– Educate residents and families on health maintenance and prevention.
– Perform routine equipment checks to ensure safety and proper function.
– Collaborate with a supportive, multidisciplinary care team.

Qualifications:
– Active LPN license from any U.S. state.
– Previous LPN experience preferred.
– Current BLS/CPR certification.
– Strong communication, compassion, and teamwork skills.
– Excellent attention to detail and organizational abilities.
