Job Type: Full-Time, Permanent

Schedule: 6 PM – 6 AM, 3 days a week with every other weekend

Job Summary:
We’re seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our team in Hubbell, MI. This full-time role offers a consistent 3-day workweek with great work-life balance and a supportive environment. You’ll provide quality nursing care to adult and geriatric residents in a very low acuity setting.

Responsibilities:
– Medication Administration: Safely prepare and administer prescribed medications; monitor effects and document appropriately.
– Treatment Delivery: Perform ordered treatments and procedures accurately while maintaining infection control standards.
– Charting & Documentation: Maintain up-to-date, accurate, and thorough records in compliance with facility and regulatory guidelines.
– Resident Monitoring: Observe and assess residents’ physical and emotional conditions; promptly report changes in status to the RN or physician.
– Care Coordination: Collaborate with the interdisciplinary care team to ensure continuity and quality of care.
– Resident Support: Provide respectful and compassionate assistance with daily living activities as needed.
– Safety & Compliance: Adhere to facility policies, procedures, and state/federal regulations to promote a safe and compliant care environment.
– Communication: Maintain open and professional communication with residents, families, and coworkers to foster a supportive care setting.
– Team Contribution: Assist with shift reports, training, and mentoring of new staff as needed to uphold facility standards.

Qualifications:
– Active Licensed Practical Nurse (LPN) license in the state of Michigan
– Minimum 2 years of LPN experience required
– Experience in adult or geriatric care preferred
– Excellent communication and teamwork skills
