Job Title: Nurse Practitioner (NP) – Geriatrics

Introduction:
We are seeking a dedicated and clinically skilled Nurse Practitioner to join our healthcare team in Fort Wayne, IN. This role focuses on patient rounding and comprehensive direct patient care, including diagnosing and treating acute and chronic conditions, prescribing medications, and developing individualized treatment plans to support positive health outcomes.

Position Offers:
– Monday–Friday schedule (no weekends)
– Competitive compensation: $145,000–$150,000 annually

Job Responsibilities:
– Conduct patient rounds and provide direct patient care
– Diagnose and treat acute and chronic illnesses
– Develop individualized treatment plans
– Prescribe medications and therapies in accordance with state and federal regulations
– Monitor patient progress and adjust treatment plans as needed
– Collaborate with interdisciplinary healthcare team members
– Educate patients and families on health conditions and preventive care

– Strong clinical judgment and diagnostic skills
– Excellent communication and interpersonal abilities
– Ability to work independently and collaboratively
– Proficiency in electronic medical records and healthcare technology

Qualifications:
 – Valid and clean Nurse Practitioner license in the state of Indiana
– 2+ years of direct patient care experience
