Key Responsibilities

• Deliver high-quality, patient-centered care while ensuring compliance with all clinical and documentation standards.

• Perform and document start-of-care assessments, recertifications, and wound care.

• Maintain thorough and accurate charting and administrative records.

• Provide leadership and oversight to Licensed Practical Nurses (LPNs), ensuring continuity and quality of care.

Qualifications

• Active Registered Nurse (RN) license in the State of Indiana (required).

• Experience with OASIS documentation is required.

• Strong attention to detail, clinical judgment, and a patient-first mindset.

• Excellent communication and teamwork skills.
